Renewing the Electoral Roll

Every six years we need to complete a new electoral roll of the church from scratch.
Please click the link below to download a simple, easy to fill in form.
You can fill the form in online and email it to This email address is being protected from spambots. You need JavaScript enabled to view it.; or you can print it, write in it and bring it to church; or fill in a paper copy in church.

  1. If you live within the parish boundaries (for each church either Prestwood and Great Kingshill, or Great Hampden) please tick box 2A.
  2. If you don't live in the parish, please tick box 2B.
  3. Typing your name in the signature box is sufficient.
